Cardiovascular Risk Management Programme
Prevention of cardiovascular disease is a health priority. Many people have risk factors making them more likely to have a heart attack or need interventions such as angioplasty or coronary by-pass surgery. In addition, those with high cardiovascular risk are more likely to suffer from stroke.
A risk management programme is a valid and cost-effective way of minimising this chance, and promoting ways of improving general and cardiovascular health.
S&C Healthcare is an experienced provider of community cardiac rehabilitation programmes, for those who have cardiac disease. We now offer a community-based multi-component intervention for those at risk. Following on from the NHS Health Checks it is a valuable intervention in preventing heart disease and stroke. It utilises relevant recommendations within NICE guidance and can support organisations in their strategy to reduce this health burden.
The programme is lead by a cardiac nurse consultant, and the support team includes:

A typical programme is 6-12 weeks depending upon the organisations need and preference and includes:
- Baseline risk assessment and goal setting
- Baseline functional capacity assessment
- Twice weekly exercise programme
- Stress management programme
- Medication management review
- Weekly healthy eating and weight management programme
- Final risk assessment and maintenance goal setting
It can be designed to meet individual and organisationals needs.
