Chronic conditions such as coronary disease should be managed for the most part in primary care.  A significant part of the management is about empowering patients to monitor and manage their condition. The Bexley cardiac rehabilitation programme uses education as a powerful tool to help patients make the changes needed to improve their health. The programme is run from several sites in the community and has patient education at its core. This includes medicines management, dietary advice,  coaching, and motivational support.

Support Group’
The Ace of Hearts is a newly formed offering support to the people of Bexley and surrounding areas.  Their aim is to encourage families and friends to come along and learn how to support their loved ones through what can be a quite a lonely and worrying time. This enables people who are affected to exchange their experiences in a warm and friendly environment. We will meet on a monthly basis, where there will be a different guest speaker each month and plenty of information on how you can help yourself or help a loved one.  Although we are unable to give advice, we can point you in the right direction for the appropriate care you require.

Bexley Community Cardiac Rehabilitation
Bexley community cardiac rehabilitation programme has made significant advances and improvements. Audit has demonstrated the programme’s effectiveness in clinical outcomes. The higher the participation in cardiac rehabilitation, the lower the re-admission rate for those patients in the future.The participation of patients in community cardiac rehabilitation in Bexley has remained much higher than the national average and has improved yet again this year. Improvements in clinical outcomes have been achieved: anxiety and depression scores are reduced, quality of life has improved and physical activity targets have been achieved.

Saving Lives and Saving Money
At the recent BACPR annual conference in Brighton there was much emphasis on cardiac rehabilitation reducing readmissions. Because patients are empowered to self manage their condition. It was promoted as a way of saving the NHS money. One presenter suggested a 30% reduction in readmissions (assuming 65% uptake) could save the NHS millions. For our patients this is a quality of care issue and re-admissions is a quality indicator. Early and comprehensive cardiac rehabilitation should help patients on the road to recovery and keep them out of hospital

Elaine has over 30 years clinical experience and was latterly cardiac nurse consultant at Guys and St Thomas hospital

Nurse specialist – Jackie Maietta
Jackie works as a practice nurse which gives her a vast experience and is also experienced in helping patients with smoking cessation.

Senior cardiac fitness instructor – Amanda Mitchell
Amanda leads both Phase 3 and Phase 4 cardiac rehabilitation exercise and relaxation classes, bringing her vast experience and energy to empower our patients

Counselling and Motivational expert – Jonathan Spencer
Jonathan is an experienced counsellor and psychotherapist, currently studying hypnotherapy as a way to help our patients

The Ace of Hearts is a newly formed Cardiac Support Group offering support to the people of Bexley and surrounding areas. The group was formed from Phase IV Cardiac Rehab run by Exercise Lead – Amanda Mitchell. The Group identified that a cardiac support group in Bexley was required, not just for the victims of cardiovascular disease and heart conditions but for their spouses who have been affected in different ways.

We hope our education and exercise and relaxation courses along with our motivational focus will mean you will want to continue with further regular exercise and a healthy diet.  There are regular cardiac exercise classes in your area Find out more

Who do I contact to be referred to cardiac rehab?

Normally you would be referred after a cardiac event or intervention and this is usually done by the hospital that you attended, if this has not happened and you feel you need cardiac rehab please ask your GP whether you are suitable for referral. Find out more
