Chronic conditions such as should be managed for the most part in primary care.  A significant part of the management is about empowering patients to monitor and manage their condition. The cardiac rehabilitation programme uses education as a powerful tool to help patients make the changes needed to improve their health. The programme is run from several sites in the community and has patient education at its core. This includes , dietary advice,  exercise coaching, and motivational support.

The major benefit is a reduced mortality. Several analyses have found that cardiac rehabilitation reduces cardiac related mortality by as much as 25%. A 2004 study also found that participating in cardiac rehabilitation reduces the risk of having a heart attack by 28%. It also significantly redcues readmission to hospital with cardiac related problems. People who go through cardiac rehabilitation also have greater improvements in cholesterol levels and blood pressure and lower stress and smoking rates, than those who do not. In addition they have fewer episodes of cardiac ischemia (reduced blood flow to the heart); slowed development (even reversal) of buildup in the arteries of the heart; weight loss; reduced need for heart-related medication; improved physical functioning and relief from depression, fear, and anxiety.

Bexley Community Cardiac Rehabilitation
Bexley community cardiac rehabilitation programme has made significant advances and improvements. Audit has demonstrated the programme’s effectiveness in clinical outcomes. The higher the participation in cardiac rehabilitation, the lower the re-admission rate for those patients in the future.The participation of patients in community cardiac rehabilitation in Bexley has remained much higher than the national average and has improved yet again this year. Improvements in clinical outcomes have been achieved: anxiety and depression scores are reduced, quality of life has improved and physical activity targets have been achieved.

Saving lives and offering real value
At the recent BACPR annual conference in Brighton there was much emphasis on cardiac rehabilitation reducing readmissions. Because patients are empowered to self manage their condition. It was promoted as a way of offering real value for the NHS and empowering patients. One presenter suggested a 30% reduction in readmissions (assuming 65% uptake) would offer real health improvements for patients and make significant savings in NHS budgets. However for our patients this is a quality of care issue and re-admissions is a quality indicator. Early and comprehensive cardiac rehabilitation should help patients on the road to recovery and keep them out of hospital

Elaine has over 30 years clinical experience and was latterly cardiac nurse consultant at Guys and St Thomas hospital

Jackie works as a practice nurse which gives her a vast experience and is also experienced in helping patients with smoking cessation.

Counselling and Motivational expert – Jonathan Spencer
Jonathan is an experienced counsellor and psychotherapist, currently studying hypnotherapy as a way to help our patients

Who do I contact to be referred to cardiac rehab?

Normally you would be referred after a cardiac event or intervention and this is usually done by the hospital that you attended, if this has not happened and you feel you need cardiac rehab please ask your GP whether you are suitable for referral. Find out more
